Medicare's hidden drug coverage trap in 2027

Your insurer can quietly add prior authorization to medications you've taken for years. Here's how to spot it before January.

In brief

Your insurer can quietly add prior authorization to medications you've taken for years. Here's how to spot it before January. Check your 2027 formulary now for new PA, QL, or ST codes next to your drugs. The Annual Election Period closes December 7, and these two-letter codes can derail your prescriptions on January…

Two letters can change everything

Two letters can change everything

Your Medicare plan can add prior authorization (PA) to a drug you've taken for years without calling you. The only warning is two quiet letters in the formulary table most people never read.

“The change that costs money isn't on the premium line. It's two letters sitting beside a drug you've taken for years, in the formulary that comes with the notice.”

What PA, QL, and ST actually mean

PA means prior authorization. QL means quantity limit. ST means step therapy. A plan can keep your drug technically covered while attaching one of these restrictions starting January 1.

“A plan can keep your drug on its formulary, still technically covered, while attaching one of those effective January 1.”

Your January 1 refill gets blocked

You hand over a prescription you've filled a hundred times. The pharmacist runs it. Because prior authorization is now required, your plan wants your doctor to justify it first, and that hasn't happened yet.

The transition safety net is short

Plans adding prior authorization mid-stream must offer a temporary supply during the first 90 days of the year. But read carefully: that's 90 days to obtain the fill, not 90 days of medication. The bridge is shorter than most assume.

“That can mean at least one month's temporary supply, available during the first 90 days of the year, or completing an appropriate transition before January 1.”

Delays happen before and after

Federal deadlines govern how fast your plan decides on prior authorization. Delays happen while your doctor's office assembles paperwork or sends incomplete requests, and after if the answer is no and you appeal. Both mean time at the pharmacy counter.

“Neither of those is the plan sitting on a decision. Both of them are time you spend at the pharmacy counter.”

Brand-name drugs face the biggest risk

Prior authorization matters most if you take brand-name maintenance drugs like anticoagulants, biologics, newer inhalers, or GLP-1s. If everything you take is generic with no flags, the formulary is a quick check.

You have until December 7 to switch

The Annual Election Period runs October 15 through December 7. Run every prescription through Medicare.gov's Plan Finder for 2027 to compare tier placement and utilization flags drug by drug. The same medication can have different restrictions at different plans.

“The same medication can sit on Tier 3 with no restrictions at one plan and Tier 3 with PA and ST at another, at a comparable premium.”

The out-of-pocket cap has limits

The Part D out-of-pocket cap rises to $2,400 in 2027, which bounds your covered drug spending. But if a prior authorization is denied and you buy the drug yourself, that money may not count toward the cap unless the claim is later approved.

“If a PA is denied and you buy the drug yourself, that money may not count toward the cap unless the claim is later approved and processed.”

Three steps to protect yourself

First, scan your formulary for PA, QL, and ST codes beside every drug you take and mark anything new. Second, call your prescriber in early January to submit the prior authorization request before you need the refill. Third, run your full drug list through Plan Finder for 2027.

“Starting the clock early is the only part of the timeline you control.”
